Despite its prevalence, it is frequently undertreated — partly because its symptoms span multiple body systems in ways that can seem unrelated.
Symptoms: More Than Just a Jaw Problem
TMJ disorder produces a recognizable cluster of symptoms — but the full range surprises many patients:
- Jaw pain or tenderness, especially when chewing, yawning, or upon waking in the morning
- Clicking, popping, or grinding sounds when opening or closing the mouth
- Jaw locking in an open or closed position
- Ear pain or a sensation of fullness in the ear — often mistaken for an ear infection
- Tinnitus (ringing in the ears)
- Headache at the temples or across the forehead

The reason TMJ symptoms span the jaw, ear, head, and neck is not coincidence — it reflects true anatomical interconnection. The trigeminal nerve, which processes sensation from the jaw joint, shares pathways with the upper cervical nerve roots. Pain in one area influences pain perception in the others.
What Causes TMJ Dysfunction?
Several factors contribute to TMJ disorder, and most patients have more than one at play.
Bruxism — teeth grinding and clenching. Chronic grinding or clenching — which many people do during sleep without being aware of it — places sustained compressive forces on the jaw joint and creates overuse fatigue in the masseter and temporalis muscles. This is one of the most common drivers of TMJ pain, and it is why night guards are so frequently recommended.
Dental malocclusion. When the upper and lower teeth do not come together evenly, the jaw compensates — shifting to one side, loading the joint asymmetrically, or requiring the surrounding muscles to work harder than they should. Over time, this uneven mechanical environment accelerates wear on the joint.
Trauma. Direct trauma to the jaw or face can displace the articular disc within the TMJ — the small cushioning disc that sits between the bones — creating the characteristic clicking or catching sensation. Whiplash is also a significant cause of TMJ dysfunction, even without direct impact to the jaw, because the rapid head movement displaces the atlas and changes head positioning in ways that affect jaw mechanics.
Psychological stress. Stress amplifies jaw clenching, increases bruxism, and generates diffuse muscle tension throughout the head, neck, and jaw region. Many patients notice that their TMJ symptoms flare significantly during periods of high stress — even when the physical structure of the jaw joint has not changed.
Upper cervical misalignment. This is the contributor that most frequently goes unaddressed. When the atlas vertebra shifts out of its optimal position, the entire head tilts or rotates subtly. Because the lower jaw attaches to the skull, a change in skull position changes how the condyle of the mandible sits in the temporal fossa — the socket of the TMJ. Over time, this asymmetric loading damages the articular disc and the surrounding tissues, and the trigeminal nerve system that processes jaw pain becomes sensitized through its shared pathways with the irritated upper cervical nerve roots.
Why Dental Treatment Alone Is Sometimes Incomplete
Dental treatment for TMJ — night guards, splints, orthodontics, Botox injections into the masseter, and in more involved cases joint surgery — addresses the oral mechanics of the condition. These interventions have genuine value and should be part of the picture when indicated.
What dental treatment cannot do is correct the position of the atlas.
If the structural misalignment at C1 is driving the asymmetric jaw loading and trigeminal sensitization, a night guard will reduce the consequences of that misalignment during sleep — but it will not remove the misalignment itself. This is why many TMJ patients experience partial relief that plateaus, or symptoms that return as soon as the splint is removed.
